Our incredible Skilled Nursing Facility in KY is looking for a superstar Nurse Practitioner to be part of a facility-based team of medical providers who provide medical services to patients living in long-term care facilities. Qualifications:
Current, unrestricted NP or PA license in the state of IL. Board Certification. DEA licensure. Clinical background in adult, family or geriatric specialties. Experience working in a long-term care setting. Ability to work with an EMR. Ability to manage patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care. The job description of a Nurse Practitioner includes conducting physical examinations, interpreting lab tests, and overseeing patients' health. As a Nurse Practitioner, you'll have to work closely with other healthcare professionals, so you should have a team spirit and a positive attitude. Patience, problem-solving skills, and communication ability are important. Responsibilities include but are not limited to:
Examine patients and their medical records when they are admitted to the Nursing home Creating an Advanced care plan Transitional care management (if they are coming from a hospital) Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with IL Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and/or the care plan.
